Key findings
  1. More than 70% of payers and providers expect GenAI’s biggest impact to land on outcomes and experience rather than productivity, pushing demand toward agentic models that act in clinical workflows.
  2. Legislative change threatens roughly 17 million more uninsured Americans by year-end 2026, puts more than 325 hospitals at risk of closure, and drives US$31.9 billion in revenue loss to expansion-state hospitals in 2026.
  3. The US healthcare AI market grew from US$50 billion to US$56 billion in 2026, healthcare AI consulting grows at roughly a 36% CAGR, and 78% of providers already use AI in revenue cycle management.
  4. Around 67% of US covered workers, 103 million lives, now sit in self-funded plans, making self-insured employers the market’s primary growth lever.
  5. The median US hospital operating margin will slip about 20 bps in 2026, payers will escalate denials to lift the claims collection burden by 10%–15%, and cash-pay and direct contracts will collapse traditional flows.

HFS assesses that the HCP services market is leaving IT modernization behind, and that vendors winning today engage clinical and operational leadership, demonstrate quantified quadruple aim outcomes, and bring risk-sharing co-innovation models, while those still selling AI productivity to CIOs will struggle. HFS finds that AI is a multi-generational catalyst with upside for access, outcomes, and economics, but reimbursement misalignment and capital allocation inertia will keep it under-realized until bold leaders force change.
